What we track, and how we report it
Every HMC program runs on one shared platform, so we can report on the same person's path from screening to referral to follow-up instead of separate numbers per tool.
Outcomes, not just activity
Mental health literacy
Participant comfort discussing mental health increased from 28 percent to 69 percent following program participation.
Resource awareness
Knowledge of available community resources increased by more than 40 percentage points among participants surveyed.
How we measure it
Outcomes on this page come from structured research, not estimates. Programs use validated screening instruments (PHQ-9, GAD-7) with pre- and post-program testing, plus participant surveys, to measure real change rather than just counting attendance. Screening results, referrals, event attendance, and follow-up all write to one data model, so we can show a funder or county partner the actual path a resident took, from screening through referral to eventual follow-up, rather than reporting isolated counts that cannot be connected to one another.
