Health Matters Clinic runs on one shared platform connecting screening, referrals, events, volunteers, and partner organizations across LA County. Community investment in HMC returns $32 in measured value for every $1 spent, and a dollar into any part of the platform shows up in outcome reporting across all of it.

Direct program delivery
Free screenings, mental health workshops, street medicine outreach, and community events reaching Los Angeles residents who would otherwise go without.
Shared coordination infrastructure
The same backend, identity layer, and data model connecting every HMC program, so a resident's journey is trackable end to end, not siloed per tool.
Community wellness incentives
Sponsor dollars can fund Health Credits, the incentive ledger that recognizes residents for screenings, education, and volunteering. The accounting layer is live; the redemption marketplace is the current funded priority.
Outcome reporting you can use
Because every tool writes to one data model, we can report on the same person's path from screening to referral to follow-up, not vanity metrics per tool.
Ways to give
Grants and contracts
Foundation grants, county contracts, and health plan partnerships. We report against agreed outcomes using data already flowing through the platform.
Sponsorship
Event and program sponsorship with visibility to the communities you want to reach, plus a direct line to the Health Credits incentive pool.
In-kind and capacity
Space, supplies, professional services, and volunteer capacity for events and outreach.
Compliance and data
Our infrastructure is built to HIPAA and SOC 2 standards. Certification is in progress, not yet complete, and we will always tell you plainly where that stands. Clinical documentation and billing run through a separate, purpose-built EHR rather than this platform, so the reporting layer you see stays focused on program and referral outcomes.
