Project Overview

Street Medicine Outreach delivers field-based care directly to unhoused populations. Teams of clinicians, nurses, and volunteers meet patients where they live—under bridges, in encampments, or on sidewalks—offering wound care, medication, and referrals for ongoing treatment. It’s health equity in motion.
