Volunteer Application & Portal Terms of Service
Effective Date: January 1, 2025

1. Purpose of Volunteer Participation

Welcome to Health Matters Clinic (“HMC,” “we,” “our,” or “us”). These Terms of Service (“Terms”) apply to all volunteers, applicants, and users who access our Volunteer Application, Volunteer Portal, or participate in any HMC programs, events, or services.

By submitting your application or accessing the volunteer system, you agree to these Terms.

Volunteers support HMC’s mission of helping communities access free health services, mental wellness support, street medicine outreach, and educational programs.
Volunteers are not employees and are not entitled to wages, benefits, or compensation.

2. Eligibility

To volunteer with HMC, you must:

Be at least 16 years old (under 18 requires parent/guardian consent)
Submit all required waivers and acknowledgments
Provide accurate information during application
Follow all HMC guidelines, policies, and instructions

HMC may accept or decline volunteer applications at its discretion.

3. Volunteer Responsibilities

Volunteers agree to:

Complete assigned tasks to the best of their ability
Communicate promptly if unavailable
Follow all safety guidelines during events and outreach
Maintain professionalism and respect toward clients, volunteers, and staff
Protect the confidentiality of client and organizational information
Never provide medical advice unless explicitly trained and authorized

HMC may reassign or discontinue volunteer roles at any time.

4. Attendance & Inactivity Policy

Volunteers must notify their team lead if unable to attend scheduled obligations.
HMC reserves the right to deactivate volunteer accounts for:

Extended inactivity
Repeated missed tasks or meetings
Lack of communication
Conduct inconsistent with HMC standards

Volunteers may reapply during the next onboarding cycle.

5. Confidentiality & HIPAA Compliance

Volunteers may encounter sensitive information related to clients, community members, or internal operations.
Volunteers agree to:

Keep all information confidential
Not disclose any private client data
Follow all HIPAA and privacy-related training provided by HMC

Any breach may result in immediate dismissal.

6. Media, Photography & Testimonial Consent (Opt-out)

Consent is given through automatic opt-in, HMC may use photos, videos, or stories from volunteer activities for:

Program documentation
Outreach and educational materials
Social media and community updates

Consent may be withdrawn at any time by emailing privacy@healthmatters.clinic.

7. Intellectual Property

All materials, content, graphics, training guides, photographs, or digital assets created while performing volunteer duties become the property of Health Matters Clinic.

This includes:

Educational materials
Social media content
Photos and videos captured for HMC
Graphic or digital design work
Data analysis or dashboards created for HMC projects

Volunteers may request permission to retain non-confidential samples for personal portfolios.

8. Data & Communications Consent

By applying, volunteers agree to receive:

Email notifications
SMS/text message updates
Event reminders
Urgent alerts
Volunteer scheduling messages

Message and data rates may apply.
Volunteers may opt out of SMS at any time by replying STOP, but doing so may limit participation.

9. Code of Conduct

Volunteers agree to maintain a safe, inclusive, and respectful environment.
The following is prohibited:

Harassment, discrimination, or bullying
Violence or threats
Disrespectful or unsafe behavior
Unauthorized possession of client or organizational data
Drug or alcohol use during volunteer activities

Violations may result in removal.

10. Liability Release

By participating in HMC activities, volunteers acknowledge that events may involve outdoor settings, physical movement, or unpredictable conditions. Volunteers agree to:
