HMC OS™ is not a product we sell. It is the system Health Matters Clinic runs on, built because the work outgrew spreadsheets, group texts, and a shared inbox. Every tool below is live today and in daily use.

Why it exists

A referral is only worth the follow-up behind it.

A community organization can hand someone a flyer and call it a referral. Nobody learns whether that person ever got care. The gap is not compassion or effort. It is that the record stops at the table.

HMC OS closes that gap. A person met at a park screening, a volunteer trained to run it, the partner who hosted the site, and the funder who paid for it are all looking at the same event from four different seats.

This is the difference between a set of websites and an operating system. Follow a single deployment through it.

    A neighborhood asks

    A partner organization or a resident requests a deployment. Before anything is planned, a community needs assessment runs on that site. Mobile Health Outreach was built from 169 of those responses.

    The event goes out

    It appears in Event Finder™ with RSVPs open. Trained volunteers are assigned from the Volunteer Portal, where their core training is already on record.

  3. 03

    Someone gets seen

    Screening happens at the table. Check Yourself™ runs on a phone for anyone who prefers privacy. Resource Directory™ turns a result into a specific place to go.

  4. 04

    Everyone sees their view

    The partner sees their site. The volunteer sees their hours. The funder sees counts they did not have to take on trust. The person keeps a plan they can act on.

Where we are

HMC OS™ is built to HIPAA and SOC 2 standards: access control, audit logging, encryption in transit and at rest, and least-privilege service accounts with no stored keys in the deploy path.

Every record is access controlled, logged, and encrypted, whether it belongs to a person we screened or a volunteer who worked the table.

Where the evidence sits

A California 501(c)(3), EIN 85-3784250, founded 2020, LA County Social Enterprise certified, active in SAM.gov, Grants.gov, and eRA Commons.

Named in the CalMHSA Intent to Award for LACDMH Community Grants through a competitive RFP. Research published in February 2025, certified exempt from review by the UCLA Institutional Review Board.
